Care Coordinator Specialist

We are seeking an experienced Care Coordinator Specialist to join our team at Job Switch Ltd.
About the RoleThe successful candidate will be responsible for coordinating care services for patients in Islington, working closely with healthcare professionals and social care teams.
Key responsibilities include:
- Conducting thorough assessments of patient needs and developing personalized care plans
- Collaborating with multidisciplinary teams to ensure seamless care transitions
- Providing education and support to patients and their families on care options and services
In this role, you will have the opportunity to make a real difference in the lives of our patients and contribute to improving healthcare outcomes in our community.
Salary: £45,000 - £55,000 per annum, depending on experience.
RequirementsTo be considered for this position, you must have:
- A master's degree or equivalent level of experience in a relevant field
- Proven experience in care coordination or a related field
- Excellent communication and interpersonal skills
- Able to work effectively in a fast-paced environment
You should also have a deep understanding of the issues facing older adults and their families, as well as knowledge of current healthcare policies and trends.
