Cancer Care Coordinator

We are seeking a highly motivated and experienced Cancer Care Coordinator to join our team at Dr Perkins Practice. The successful candidate will play a key role in supporting the early cancer diagnosis and cancer care quality improvement work across our Primary Care Network.
Key Responsibilities- Promote prevention through education events, poster distribution, and social media advertising.
- Consult with Digital Champions to enhance online presence and engagement.
- Work with local community groups to provide patient information, awareness, and support.
- Identify at-risk populations and develop systems to improve cancer screening uptake.
- Coordinate referrals, manage safety netting processes, and ensure timely diagnoses.
- Drive personalized patient care through early phases after diagnosis and ensure referrals to key healthcare providers.
- Support practices in delivering quality improvement plans for early cancer diagnosis.
- Ensure patients receive timely Cancer Care reviews in line with national targets.
Dr Perkins Practice is a leading Primary Care Network in Oldham, comprising 11 GP practices. We work together to provide a range of extra services, prioritizing the needs of our local communities. Requirements- Experience in a patient-facing role, preferably in cancer care or primary care.
- Essential qualifications: GCSE grade A-C in English and Maths.
- Desirable qualifications: NVQ3 in Health & Social Care.
- Excellent interpersonal, communication, and organizational skills.
- Ability to work in high-pressure situations and adapt to shifting priorities.
- Competent administrative and IT skills.
