About the Role

We are seeking a highly motivated and experienced Cancer Care Coordinator to join our team at Dr Perkins Practice. As a key member of our Primary Care Network, you will play a vital role in supporting the early cancer diagnosis and cancer care quality improvement work across our network of 11 GP practices in central Oldham.

Key Responsibilities
  • Promote prevention through education events, poster and leaflet distribution, and social media advertising.
  • Work with local community groups to provide patient information, awareness, and support.
  • Identify at-risk populations through the QOF register and develop systems to improve cancer screening uptake.
  • Co-ordinate referrals and manage safety netting processes to ensure timely diagnosis and treatment.
  • Drive up standards of personalised patient care by involving patients in early phases after diagnosis and ensuring referral to other key healthcare providers.

Person Specification
  • Experience in a patient/customer-facing role.
  • Essential qualifications: GSCE grade A-C in English and Maths.
  • Desirable qualifications: NVQ3 in Health & Social Care.
  • Excellent interpersonal, communication, and organisational skills.
  • Ability to work in high-pressure situations and adapt to shifting priorities.
