About the Role

As a key member of our Population Health Management (PHM) team, you will play a vital role in supporting the delivery of our Frailty Programme, which aims to provide proactive and holistic care to individuals with mild to moderate frailty.

Key Responsibilities
  • Manage a caseload of patients with mild and moderate frailty, providing personalized support and care coordination to ensure they receive the necessary services and resources.
  • Work closely with multiple professionals from across the sectors to coordinate patient care, ensuring seamless transitions and effective communication.
  • Support the ongoing case management of patients, including regular check-ins, response to significant events, and review of patient-centered care plans.
  • Develop and maintain relationships with patients, families, and caregivers, providing education and support to empower them to make informed decisions about their care.
  • Contribute to the implementation and evaluation of the PHM programme, providing feedback and insights to inform programme development and improvement.
Requirements
  • Relevant health and social care experience, preferably in a Primary Care setting.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with patients, families, and healthcare professionals.
  • Ability to work effectively in a team environment, with a focus on collaboration and coordination.
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
  • Knowledge of local health and care infrastructure, with the ability to navigate and access relevant services and resources.
