About the Role

We are seeking a highly organized and communicative Care Coordinator to join our team at Gtd Healthcare. As a key member of our Urgent Care team, you will be responsible for providing exceptional patient care and coordination services to patients and healthcare professionals across the Oldham area.

Key Responsibilities
  • Deliver a high-quality healthcare coordination service to patients and healthcare professionals.
  • Accurately record patient data on clinical systems and direct patients to the most appropriate service or pathway.
  • Undertake appointment booking and coordination across multiple healthcare services.
  • Liaise with the Senior Care Coordinator and Clinical Shift Lead to ensure smooth patient flow and minimize waiting times.
  • Coordinate follow-up care as directed by the senior care coordinator or clinical team.
  • Liaise with the multi-disciplinary team to ensure seamless service delivery.
  • Collaborate with NHS 111 to address out-of-area patients, duplicate calls, or incomplete information.

  • Ability to work effectively in a multi-disciplinary environment.

  • Ability to maintain confidentiality and work under pressure.
