Care Coordinator

We are seeking a highly skilled and compassionate Care Coordinator to join our team at Hope Citadel Healthcare CIC. As a key member of our Population Health Management (PHM) team, you will play a vital role in supporting the delivery of our Frailty Programme, which aims to provide proactive and holistic care to individuals with mild to moderate frailty.
Key Responsibilities- Manage a caseload of patients with mild and moderate frailty, providing personalized support and care coordination to ensure they receive the necessary services and resources.
- Work closely with multiple professionals from across the sectors to coordinate patient care, ensuring seamless communication and collaboration.
- Support the ongoing case management of patients, including regular check-ins, response to significant events, and review of patient-centered care plans.
- Develop and maintain effective working relationships with GPs, district nurses, therapists, physios, adult social care, and other PCN colleagues.
- Act as a conduit for patients within the PCN, liaising with healthcare professionals to ensure timely and effective care.
- Support the implementation of the PHM programme, working within governance structures and providing feedback and iteration of the model.
- Maintain accurate records of patient journeys, including services referred to and health inequalities identified.
- Relevant health and social care experience at AfC Band 3 or 4 or equivalent, with a strong understanding of the NHS and social care systems.
- Experience of administrative duties, including data entry and record-keeping.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
- Ability to work independently and as part of a team, with a strong focus on patient-centered care.
- Knowledge of local health and care infrastructure, with the ability to navigate and access relevant services and resources.
