About the Role

We are seeking a highly organized and communicative Care Coordinator to join our team at Gtd Healthcare. As a key member of our Urgent Care team, you will be responsible for coordinating patient care and ensuring seamless communication between healthcare professionals and patients.

Key Responsibilities
  • Deliver high-quality care coordination services to patients and healthcare professionals.
  • Accurately record patient data and direct patients to the most appropriate service or pathway.
  • Book appointments across various healthcare services and ensure timely referrals.
  • Liaise with the Senior Care Coordinator and Clinical Shift Lead to address operational issues and minimize waiting times.
  • Coordinate follow-up care as directed by the Senior Care Coordinator or clinical team.
  • Liaise with the multi-disciplinary team to ensure smooth service delivery.
  • Collaborate with NHS 111 to address out-of-area patients, duplicate calls, or incomplete information.
