Key Responsibilities- Deliver high-quality care coordination services to patients and healthcare professionals.
- Accurately record patient data and direct patients to the most appropriate service or pathway.
- Book appointments across multiple healthcare services and ensure timely referrals.
- Liaise with the Senior Care Coordinator and Clinical Shift Lead to address operational issues and minimize waiting times.
- Coordinate follow-up care and ensure smooth communication with the multi-disciplinary team.
- Liaise with NHS 111 regarding out-of-area patients, duplicate calls, or incomplete information.
- Good standard of education.
- Excellent organization and communication skills.
- Ability to work as part of a team and under pressure.
- Polite and friendly telephone manner.
- Experience of reception or call handling duties and working with the public.
