About the Role

We are seeking a highly motivated and experienced Cancer Care Coordinator to join our team at Dr Perkins Practice. The successful candidate will play a key role in supporting the early cancer diagnosis and cancer care quality improvement work within our Primary Care Network.

Key Responsibilities
  • Promote cancer prevention through education events, poster distribution, and social media advertising.
  • Work with local community groups to provide patient information, awareness, and support.
  • Identify at-risk populations through the QOF register and develop systems to improve cancer screening uptake.
  • Coordinate referrals and manage safety netting processes to ensure timely diagnosis and treatment.
  • Collect and analyze data to improve coding, screening, and health inequalities.
  • Support practices in delivering their quality improvement plans for early cancer diagnosis.

Requirements
