Community Nurse

We are seeking a skilled and dedicated Community Nurse to join our team at Central London Community Healthcare NHS Trust. As a Community Nurse, you will play a vital role in delivering high-quality patient care in the community setting.
Key Responsibilities- Provide skilled nursing care to patients in their homes, based on principles of best practice and clinical evidence.
- Manage chronic diseases, end-of-life care, continence promotion, and wound management.
- Act as a Named Nurse with the support and supervision of the DN Team Lead.
- Conduct comprehensive assessments of patients' physical, psychological, and social care needs.
- Develop and implement individualized care plans that promote self-care and prevent unnecessary hospital admissions.
- Registered Nurse (RN) qualification.
- NMC Registration.
- Experience of working in a community setting, preferably with people with complex health needs.
- Ability to work effectively in a team and communicate well verbally and in writing.
