About the Role

We are seeking a skilled and dedicated Community Nurse to join our team at Central London Community Healthcare NHS Trust. As a Community Nurse, you will play a vital role in delivering high-quality patient care in the community setting.

Key Responsibilities
  • Provide skilled nursing care to patients in their homes, based on principles of best practice and clinical evidence.
  • Manage chronic diseases, end-of-life care, continence promotion, and wound management.
  • Act as a Named Nurse with the support and supervision of the DN Team Lead.
  • Conduct comprehensive assessments of patients' physical, psychological, and social care needs.
  • Develop and implement individualized care plans that promote self-care and prevent unnecessary hospital admissions.
Requirements
  • Registered Nurse (RN) qualification.
  • NMC Registration.
  • Experience of working in a community setting, preferably with people with complex health needs.
  • Ability to work effectively in a team and communicate well verbally and in writing.
