Job Title: Bank Band 5 Community Nurse

We are seeking a skilled and compassionate Bank Band 5 Community Nurse to join our team at Central London Community Healthcare NHS Trust. As a Community Nurse, you will play a vital role in providing high-quality nursing care to patients in their own homes and community settings.

About the Role:

As a Community Nurse, you will be responsible for delivering skilled nursing care to patients with a range of health needs, including chronic conditions, end-of-life care, and wound management. You will work closely with patients, their families, and other healthcare professionals to develop and implement personalized care plans that promote independence, self-care, and optimal health outcomes.

Key Responsibilities:
  • Provide skilled nursing care to patients in their own homes and community settings
  • Conduct comprehensive assessments of patients' physical, psychological, and social care needs
  • Develop and implement individualized care plans that promote self-care and independence
  • Work collaboratively with patients, families, and other healthcare professionals to coordinate care and achieve optimal health outcomes
  • Act as a patient advocate and facilitate their choices regarding nursing care
Requirements:
  • Registered Nurse (RN) with current NMC registration
  • Recent clinical nursing experience in a community setting
  • Knowledge of out-of-hospital care and the role of the Community Nurse
  • Ability to work autonomously and as part of a multidisciplinary team
