Job Title: Community Nurse - Band 5

Job Summary:

We are seeking a highly skilled and motivated Community Nurse to join our team. As a Community Nurse, you will be responsible for providing high-quality nursing care to patients in their homes and community settings. You will work closely with other healthcare professionals to deliver seamless care and support to our patients.

Key Responsibilities:

  • Provide skilled nursing care to patients in their homes and community settings.
  • Conduct holistic assessments of patients' health needs and develop individualized care plans.
  • Work collaboratively with other healthcare professionals to deliver comprehensive care and support to patients.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care and support.
  • Participate in audits and research projects to improve patient care and outcomes.

Requirements:

  • Registered Nurse (RN) with current NMC registration.
  • Recent clinical nursing experience in a community setting.
  • Knowledge of out-of-hospital care and the role of the Community Nurse.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
