Job Title: Community Nurse Band 5

We are seeking a skilled and compassionate Community Nurse to join our team at Central London Community Healthcare NHS Trust. As a Community Nurse, you will play a vital role in providing high-quality nursing care to patients in their own homes and local clinics.

About the Role

As a Community Nurse, you will be responsible for delivering skilled nursing care to patients with a range of health needs, including chronic disease management, end-of-life care, and wound management. You will work closely with patients, their families, and other healthcare professionals to develop and implement personalized care plans that promote independence and self-care.

Key Responsibilities
  • Provide skilled nursing care to patients in their own homes and local clinics
  • Conduct comprehensive assessments of patients' physical, psychological, and social care needs
  • Develop and implement individualized care plans that promote independence and self-care
  • Work closely with patients, their families, and other healthcare professionals to coordinate care
  • Act as a patient advocate and facilitate their choices regarding nursing care
Requirements
  • Registered Nurse (RN) with NMC registration
  • Recent clinical nursing experience negotiating and liaising within a multidisciplinary team
  • Knowledge of out-of-hospital care and the role of the Community Nurse
