As a Support Worker, you will be responsible for supporting and enabling individuals to live their best life. This includes delivering person-centered care aligned with our person-led support delivery model.

Our ideal candidate will have experience in providing support for young people with complex needs, building meaningful connections within their community, and enabling them to live their best life.

You will work alongside a dedicated team who care deeply about their work, the people they support, and their colleagues, in a welcoming and inclusive environment where everyone is valued and respected.

Your responsibilities will include:

  • Delivering high-quality care by utilizing your knowledge, skills, and adherence to legislation, best practices, and our policies and procedures.
  • Engaging with individuals to help them maintain their skills and receive personalized care.
  • Contributing to the development and maintenance of relationships and connections within their local community.
  • Completing and maintaining Support Plans and other necessary information that supports our people and ensures effective support.
  • Notifying the Team Leader/Senior Support Worker of any changes you observe in the needs of the individuals we support.
  • Actively promoting and maintaining the safety of those in our care.
  • Helping individuals to maintain and develop relationships with family, friends, and other important people in their lives.

This role requires a salary of £28,000 per annum and offers opportunities for career growth and professional development.
