Clinical Lead for Crisis Resolution and Home Treatment Teams

We are seeking a highly skilled Clinical Lead to oversee the delivery of high-quality patient-centred care as part of our Crisis Resolution and Home Treatment Teams.
Key Responsibilities- Lead clinical teams in providing effective, safe, and efficient care to patients
- Collaborate with community, inpatient, liaison, and perinatal teams to ensure seamless care transitions
- Develop and implement care plans that prioritize patient well-being and outcomes
- Provide direct assessments and consultations to patients, including those requiring complex case management
- Lead reviews of patients admitted to crisis houses and centres, ensuring timely interventions and support
