Community Staff Nurse

About the Role
We are seeking a highly motivated and enthusiastic Community Staff Nurse to join our team at Central and North West London NHS Foundation Trust. As a Community Staff Nurse, you will be responsible for delivering high-quality nursing care to patients in their own homes, working closely with the multidisciplinary team to provide comprehensive care.
Key Responsibilities
- Deliver high-quality nursing care to patients in their own homes, including wound care, medication management, and palliative care.
- Work closely with the multidisciplinary team to provide comprehensive care, including GP and practice nurse collaboration.
- Develop and maintain effective communication with patients, carers, and other healthcare professionals.
- Record accurate details of care given, observations, and monitoring of patients, and assist in general record keeping and documentation.
- Hold a full UK driver's license and have use of a car for business purposes or have independent means of transport to ensure timely delivery of patient care.
Requirements
- NMC registration.
- Excellent communication skills - written and verbal.
- Numeracy skills required for the safe administration of medicines.
- Able to develop and maintain communication with people about difficult matters and/or in difficult situations.
- Sound IT skills.
- Support equality and value diversity.
- Understanding of concept of risk management within a community setting.
