About the Role

We are seeking a skilled and dedicated Community Staff Nurse to join our team at Central and North West London NHS Foundation Trust. As a Community Staff Nurse, you will be responsible for providing high-quality nursing care to patients in their own homes, working closely with multidisciplinary teams to deliver comprehensive care.

Key Responsibilities

  • Provide skilled nursing care to patients in their own homes, including wound care, medication management, and palliative care.
  • Work collaboratively with multidisciplinary teams, including GPs, practice nurses, and other healthcare professionals, to deliver comprehensive care.
  • Conduct thorough assessments of patients' needs, develop care plans, and implement interventions to improve patient outcomes.
  • Communicate effectively with patients, carers, and other healthcare professionals to ensure seamless care delivery.
  • Participate in quality improvement initiatives to enhance patient care and service delivery.

Requirements

  • NMC registration as a Registered Nurse.
  • Experience of working in a community setting, preferably in a district nursing role.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, carers, and other healthcare professionals.
  • Ability to work autonomously and as part of a team, with a flexible approach to working hours.
  • Sound knowledge of current health service issues and an understanding of primary care.
