About the Role
We are seeking a highly motivated and enthusiastic Band 5 Community Staff Nurse to join our team in Hillingdon. As a Community Staff Nurse, you will be responsible for providing high-quality nursing care to patients in their own homes, working closely with the multidisciplinary team to deliver a comprehensive service.
Key Responsibilities
- Provide nursing care to patients in their own homes, including wound care, medication management, and palliative care.
- Work collaboratively with the multidisciplinary team to deliver a comprehensive service, including GP and practice nurse.
- Develop and maintain effective communication with patients, carers, and other healthcare professionals.
- Record accurate details of care given, observations, and monitoring of patients.
Requirements
- NMC registration.
- Excellent communication skills, written and verbal.
- Numeracy skills required for the safe administration of medicines.
- Able to develop and maintain communication with people about difficult matters and/or in difficult situations.
- Sound IT skills.
- Support equality and value diversity.
- Understanding of concept of risk management within a community setting.
