About the Role

We are seeking a highly motivated and enthusiastic Community Staff Nurse to join our team in Hillingdon. As a Band 5 Community Staff Nurse, you will be responsible for delivering high-quality nursing care to patients in their own homes.

Key Responsibilities
  • Provide autonomous nursing care to patients with a range of needs, including diabetic management, wound care, and palliative care.
  • Work collaboratively with the multidisciplinary team to deliver a comprehensive service to patients.
  • Develop and maintain effective communication with patients, their carers, and other healthcare professionals.
  • Contribute to the development of the district nursing service, including identifying areas for improvement and implementing changes.
Requirements
  • NMC registration as a nurse.
  • Experience of working in a community setting, preferably in a district nursing role.
  • Excellent communication and interpersonal skills.
  • Ability to work autonomously and as part of a team.
  • Sound knowledge of current health service issues and policies.
