We are seeking a highly skilled and compassionate Care Coordinator to join our team at Home Instead. As a Care Coordinator, you will play a vital role in delivering high-quality care to our clients and supporting our Care Professionals in their work.
Key Responsibilities
- Conduct care consultations and create personalized support plans to ensure the best possible care for our clients.
- Collaborate with clients and their families to understand their needs and preferences.
- Work closely with our Care Professionals to ensure they have the necessary support and resources to deliver exceptional care.
- Maintain accurate records of client interactions and care plans.
- Participate in regular team meetings and contribute to the development of our care services.
- Support the growth and development of our Care Professionals through training and mentoring.
- Ensure compliance with our Equality, Diversity, and Equal Opportunities Policy.
Requirements
- Level 3 NVQ in Health and Social Care or equivalent.
- Experience in the care sector delivering a wide range of personal care services.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Knowledge of legislation and regulations specific to Health and Social Care.
- Full driving license and means of transport if required.
