Job Title: Care Coordinator
Job Summary:
Cera Care Ltd is seeking a skilled Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in ensuring the delivery of high-quality care services to our clients. You will be responsible for effectively matching Care Workers with clients based on their care needs, while also considering proximity whenever possible.
Key Responsibilities:
- Manage the delivery of care services to meet client needs and expectations.
- Coordinate the allocation of Care Workers to clients, ensuring the right skills and experience are matched to the client's requirements.
- Conduct regular care assessments and reviews to ensure the quality of care is maintained.
- Develop and maintain relationships with clients, their families, and Care Workers to ensure a seamless care experience.
- Collaborate with the Branch Manager and other team members to achieve business objectives and improve care services.
- Stay up-to-date with relevant legislation, regulations, and industry standards to ensure compliance.
Requirements:
- NVQ/SVQ 2 or 3 qualification (desirable)
- Registration with the SSSC (Scotland) or SCW (Wales)
- At least 2 years' experience in a care coordination role, preferably in a Domiciliary Care Agency
- Knowledge of CQC/CI/CIW Compliance
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Flexibility to work varied shifts and adapt to changing priorities
