Job Title: Care Coordinator - Team Lead

Job Summary:

Community Care Direct Ltd is seeking a highly skilled and experienced Care Coordinator to lead our team of care professionals. As a Care Coordinator - Team Lead, you will be responsible for managing the day-to-day operations of our care team, ensuring that our service users receive safe, compliant, and high-quality care in their own homes.

Key Responsibilities:

  • Conduct thorough risk assessments of service users, including their care environment and equipment.
  • Develop and implement new care packages in line with company and regulatory requirements.
  • Maintain confidentiality of service users and staff at all times.
  • Assess, reassess, and amend service user risk assessments and care plans as needed.
  • Participate in regular meetings with the care team and attend service user reviews as requested.
  • Manage the on-call rota and provide support to the care team as needed.

Requirements:

  • Strong understanding of CQC regulations and domiciliary care standards.
  • Excellent communication and interpersonal skills.
  • Ability to work flexibly and adapt to changing circumstances.
  • Relevant Level 3 qualification or willingness to obtain within 6 months.
  • Valid car driver's license.
