Job Summary:
Community Care Direct Ltd is seeking a highly skilled and experienced Care Coordinator to join our team as a Team Lead. As a Care Coordinator, you will be responsible for managing the day-to-day operations of our care team, ensuring that our service users receive safe, compliant, and high-quality care in their own homes.
Key Responsibilities:
- Conduct thorough risk assessments of service users, including their care environment and equipment.
- Develop and implement new care packages in line with company and regulatory requirements.
- Maintain confidentiality of service users and staff at all times.
- Assess, reassess, and amend service user risk assessments and care assessments in response to changes.
- Participate in regular meetings with the care team and attend service user reviews as requested by the care manager.
- Manage the on-call rota and provide support to the care team as needed.
Requirements:
- Strong understanding of CQC regulations and domiciliary care standards.
- Excellent communication and interpersonal skills.
- Ability to work flexibly and adapt to changing circumstances.
- Relevant Level 3 qualification or willingness to undertake within 6 months of appointment.
- Valid car driver's license.
