Job Title: Mental Health and Social Care Coordinator

We are seeking a skilled Mental Health and Social Care Coordinator to join our team. As a Mental Health and Social Care Coordinator, you will be responsible for coordinating the delivery of mental health and social care services, ensuring that individuals receive the support they need to achieve their goals.

The ideal candidate will have experience in mental health, social care, or a related field, with a strong understanding of the challenges faced by individuals with mental health issues. You will be required to work collaboratively with colleagues, stakeholders, and service users to develop and implement effective care plans.

A competitive salary of £35,000 - £45,000 per annum (dependent on experience) is offered for this role, along with a range of benefits and opportunities for professional development.

Responsibilities:
  • Coordinate the delivery of mental health and social care services, ensuring that individuals receive the support they need to achieve their goals.
  • Develop and implement effective care plans in collaboration with colleagues, stakeholders, and service users.
  • Work collaboratively with multidisciplinary teams to ensure seamless care delivery.
  • Maintain accurate records and reports, adhering to regulatory requirements and organisational policies.
Requirements:
  • Degree in Mental Health or a related field.
  • Post-qualification experience in mental health, social care, or a related field.
  • Strong understanding of the challenges faced by individuals with mental health issues.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Able to work collaboratively with diverse stakeholders and service users.
