About the RoleThis exciting opportunity allows you to make a real difference in the lives of young people with mental health needs. As a Senior Care Coordinator, you will be responsible for providing high-quality care and support to individuals with complex needs.
You will work closely with our multi-disciplinary team to develop and implement individualized care plans, ensuring that each person receives the best possible care and support. Your excellent communication and interpersonal skills will enable you to build strong relationships with patients, families, and colleagues.
