About the Role:


We are seeking an experienced Senior Community Care Coordinator to join our Discharge to Assess team. As a key member of our multidisciplinary team, you will be responsible for coordinating high-quality care services for patients referred from acute general hospitals.


Key Responsibilities:



  • Clinically lead and coordinate care delivery for patients with complex needs
  • Collaborate with healthcare professionals to develop personalized care plans
  • Monitor patient progress and adjust care plans as needed
  • Work closely with community teams to ensure seamless transitions of care
  • Develop and maintain strong relationships with patients, families, and caregivers

Requirements:



  • Registered Nurse (Adult) with 2+ years of experience in a community setting
  • Strong clinical skills and knowledge of Continuing Healthcare and assessment processes
  • Excellent communication and interpersonal skills
  • Ability to work autonomously and manage competing priorities
  • Valid UK Driving License
