About the Role

We are seeking a skilled clinician to join our community care team as a Home Visiting Clinician. The successful candidate will provide holistic care for individuals in their homes, assessing, diagnosing, and managing their conditions to achieve the best possible outcomes.

Key Responsibilities

  • Undertake comprehensive geriatric assessments, including medication reviews and advanced care planning where appropriate.
  • Diagnose and manage acute and chronic conditions, utilising and referring to community services as needed.
  • Communicate findings and management plans with patients, relatives, and referring clinicians.
  • Provide safety netting and worsening advice for patients and relatives.
  • Ensure comprehensive, timely, and accurate documentation.

We have a strong focus on health promotion and personalised care, supporting patients to make informed decisions about their health and social care.

Person Specification

  • Regulated Health Care professional with current registration (NMC/HCPC Registered).
  • Minimum of three years post-registration experience with demonstrable use of diagnostic skills.
  • Postgraduate certificate or diploma (nurses only).

Desirable Qualifications and Skills

  • Recognised qualification in physical assessment and clinical reasoning or equivalent.
  • Experience of working in Primary Care.
  • Special interest in frailty and older people.
