We are currently seeking an experienced Care Transition Specialist to join our team. This full-time position will be based in one of our care homes, where you will work closely with our Home Manager and other professionals to ensure seamless transitions for our residents.

The ideal candidate will have a strong background in nursing, with experience in caring for residents with advanced dementia. You will need to possess excellent communication skills, a passion for your work, and a commitment to delivering high-quality care.

In this role, you will conduct thorough pre-admission assessments, develop individualized care plans, and work with other professionals to identify training needs. You will also be responsible for ensuring that all input, advice, or requirements from other services are implemented on admission.

We offer a competitive salary range of $60,000 - $80,000 per annum, depending on experience. As a valued member of our team, you can expect:
