Job Title: Community Nurse Specialist

About the Role:

We are seeking an experienced Community Nurse Specialist to join our Discharge to Assess team. As a key member of our team, you will be responsible for providing high-quality nursing care to patients in their own homes, ensuring a safe and timely hospital discharge.

Key Responsibilities:

  • Conduct comprehensive assessments of patients' needs and develop individualized care plans.
  • Provide education and support to patients, families, and carers to promote independence and self-management.
  • Work collaboratively with other healthcare professionals to ensure seamless care transitions.
  • Participate in the development and implementation of care pathways and protocols.

Requirements:

  • Registered Nurse (Adult) with a minimum of 2 years' experience in a community setting.
  • Excellent communication and interpersonal skills.
  • Ability to work autonomously and as part of a team.
  • Knowledge of Continuing Healthcare and the assessment process.
