Community Nurse Specialist

We are seeking an experienced Registered Nurse to join our Discharge to Assess team as a Community Nurse Specialist. This is an exciting opportunity to work in a dynamic and multi-skilled team, providing high-quality nursing care to patients in their own homes.
Key Responsibilities- Provide holistic, evidence-based care to patients in accordance with national and organisational policies and procedures.
- Work autonomously and unsupervised in the community setting, recognising the risks associated with lone working and taking proactive steps to reduce those risks.
- Assess, plan, implement, and evaluate patient care on a day-to-day basis, as part of a multi-professional team.
- Develop and maintain effective communication networks with other members of the wider health and social care community and other agencies.
- Act as a patient advocate, ensuring individual needs, preferences, and choices are delivered by the service.
- 2+ years of recent experience as a Band 5 Nurse in an acute or community setting.
- Ability to demonstrate transferable skills, including time management, environmental stress management, and adaptability.
- Relevant clinical experience and competence, including medicines management and complex care coordination.
- Excellent interpersonal skills, ability to build and maintain relationships with other health and social care professionals, patients, carers, and the public.
- Excellent written and verbal communication skills.
- Ability to work autonomously without supervision and adapt to the demands of a constantly changing environment.

- Access and funding for training, including Care Certificate, Assistant Practitioners Course, and Scholarship Into Nurse Training.
