About the Role

We are seeking an experienced Registered Nurse to join our Discharge to Assess team as a Community Nurse Specialist. This is an exciting opportunity to work in a dynamic and multi-skilled team, providing high-quality nursing care to patients in their own homes.

Key Responsibilities
  • Provide holistic, evidence-based care to patients in accordance with national and organisational policies and procedures.
  • Work autonomously and unsupervised in the community setting, recognising the risks associated with lone working and taking proactive steps to reduce those risks.
  • Assess, plan, implement, and evaluate patient care on a day-to-day basis, as part of a multi-professional team.
  • Develop and maintain effective communication networks with other members of the wider health and social care community and other agencies.
  • Act as a patient advocate, ensuring individual needs, preferences, and choices are delivered by the service.
Requirements
  • 2+ years of recent experience as a Band 5 Nurse in an acute or community setting.
  • Ability to demonstrate transferable skills, including time management, ability to manage environmental stress, and excellent interpersonal skills.
  • Relevant clinical experience and competence, including medicines management, complex care coordination, and supporting self-care and independence.
