We have an exciting opportunity for an experienced Mental Health Adult Transitions Social Worker to join our team at Proud to Care Devon.

About the Role:

You will work closely with young people, families, and a multidisciplinary team to ensure a smooth, person-centred transition experience that promotes independence, dignity, and holistic well-being.

Key Responsibilities:
  • To engage with young people with needs relating to their mental health, ensuring that independent living skills are promoted and maximised.
  • To work collaboratively with young people, families, and the multidisciplinary team to ensure a seamless transition from children's to adult services.
Requirements:
  • An experienced and qualified Social Worker (CQSW, DipSW, BA Hons, MA or equivalent) who is registered to practice with Social Work England.
  • A comprehensive working knowledge of the issues currently impacting on social care delivery.
  • A current full driving licence and access to a vehicle due to the requirement to travel significant distances.
What We Offer:
  • A competitive salary of £40,000 - £50,000 per annum, depending on experience.
  • 26 days annual leave, plus Bank Holidays, rising incrementally to 31 days after 5 years' service.
  • Flexible working opportunities, including part-time hours, flexitime, compressed working hours, and homeworking.
  • An opportunity to enter a generous Local Government Pension Scheme.
