Community Health Worker

Premier Community is committed to delivering exceptional care to our clients, and we are seeking an experienced Community Health Worker to join our team.
This role offers a competitive hourly rate of £14.40ph to £16.00ph, paid on demand, and guaranteed contracted hours for minimum earnings. You will also receive 5.6 fully paid weeks of holiday, company pension, and access to fully funded qualifications in Health and Social Care.
As a Community Health Worker, you will be responsible for delivering high-quality care to our clients in their own homes, promoting independence, and supporting them with daily living activities.
Key Responsibilities:- Providing personalized support to clients, including assistance with daily living activities
- Assisting with medication management and administration
- Preparing and serving meals and snacks
- Supporting clients with mobility, transfers, and other daily needs
- A full UK driving license
