Community Health Nurse Specialist

We are seeking a skilled nurse to join our Chesterfield Central Community Nursing Team on a 12-month secondment maternity leave cover.

As a valued member of our team, you will provide evidence-based nursing care to patients in their homes.

You will assess, plan, implement, and evaluate individualised patient care, working closely with Integrated Community Teams/GP surgeries.

The role requires flexibility, excellent communication skills, and the ability to work independently as part of a supportive team.

Main Responsibilities
  • Deliver high-quality nursing care to patients in their homes.
  • Assess and develop individualised patient care plans.
  • Collaborate with healthcare professionals to ensure seamless care delivery.
