Job Description

We are seeking a highly motivated and empathetic Community Health Worker to join our team at Premier Community. In this role, you will provide personalized support and care to our clients, enhancing their overall well-being and quality of life.

Key Responsibilities
  • Deliver high-quality care and support to clients in their homes
  • Collaborate with multidisciplinary teams to develop and implement client-centered care plans
  • Communicate effectively with clients, families, and healthcare professionals
  • Record visits using our digital care planning system
Requirements

To excel in this role, you will need:

  • A full UK driving license
  • High levels of empathy and understanding for vulnerable individuals
  • A professional approach to training and ongoing development
