This exciting opportunity has arisen for a skilled nurse to join our Chesterfield Central Community Nursing Team on a 12-month secondment maternity leave cover.

As a key member of our team, you will make a positive impact on patients' lives by providing high-quality nursing care in their homes.

You will work closely with Integrated Community Teams/GP surgeries, assessing, planning, implementing, and evaluating individualised patient care.

This role requires excellent communication skills, flexibility, and the ability to work independently as part of a supportive team.

Key Responsibilities
  1. Provide nursing care to patients in their homes.
  2. Develop and deliver individualised patient care plans.
  3. Collaborate with healthcare professionals to ensure effective care delivery.
