Job Title: Care and Support Specialist

About the Role:

We are seeking a highly motivated and compassionate Care and Support Specialist to join our team at Greenfields Close. As a key member of our staff, you will play a vital role in providing high-quality, enabling care to individuals with learning disabilities, autism, mental health issues, physical disabilities, and complex care needs.

Key Responsibilities:

- Implement person-centered support plans to promote independence and well-being
- Develop flexible, engaging activity plans to promote autonomy and personal growth
- Support individuals in maintaining healthy lifestyles and managing their health needs
- Oversee medication processes and support individuals in self-administration
