**Job Title:** District Nursing Team Coordinator

Location: Central London, UK

About the Role:

We are seeking an experienced and motivated District Nursing Team Coordinator to join our dynamic Brent Community Nursing Team. As a key member of our team, you will be responsible for coordinating the work of a team of nurses and healthcare assistants in delivering high-quality patient care in a community setting.

The Ideal Candidate:

We are looking for a qualified Registered General Nurse with current UK NMC registration and a Specialist Practitioner qualification in District Nursing (or working towards it). You should have proven experience in community nursing with a focus on complex care, team coordination, and multidisciplinary collaboration.

Key Responsibilities:

  • Coordinate the work of a team of nurses and healthcare assistants in providing high-quality patient care
  • Develop and implement effective care plans, focusing on health needs assessments and coordinated discharge planning
  • Collaborate closely with GPs, social care, and other health and voluntary services to provide seamless, integrated care
  • Promote service development by monitoring care quality, identifying opportunities for improvement, and supporting continuous professional development within the team

What We Offer:
