District Nursing Team Member

Job Description: We are seeking a qualified and experienced Community Health Nurse to join our team. As a key member of our District Nursing Team, you will be responsible for delivering high-quality nursing care to patients in their homes and community settings.

Central London Community Healthcare NHS Trust provides community health services to over 2 million people across eleven London boroughs and Hertfordshire. Our team is dedicated to delivering excellent patient care and promoting independence and self-care for patients.
Key Responsibilities:
- Deliver skilled nursing care to patients with long-term conditions, including chronic disease management, end-of-life care, and wound management.
- Act as a Named Nurse, taking responsibility for the holistic health needs assessment, care planning, and evaluation of patient care.
- Work collaboratively with multidisciplinary teams to ensure seamless care transitions and promote independence and self-care for patients.
- Utilize evidence-based practice to provide person-centered care, promoting the physical, psychological, and social well-being of patients.
Requirements:
- NMC Registration as a Registered Nurse (RN).
- Recent clinical nursing experience negotiating and liaising within a multidisciplinary team.
- Knowledge of out-of-hospital care and the role of the Community Nurse.
- Awareness of the management and use of data in the NHS setting.
Estimated Salary: £42,000 - £55,000 per annum, dependent on experience.
