About the Job

We are recruiting for a Home Care Assistant - Powys to join our team in providing high-quality care services to individuals living within their homes in the local community.

The successful candidate will be responsible for delivering person-centred care that promotes independence, well-being, and community presence.

Key Responsibilities:

  • Implementing Personal Plans to meet individual outcomes in regaining skills, independence, and ongoing support requirements
  • Providing front-line care and support to individuals living within their homes
  • Promoting independence to support individuals to achieve their identified personal outcomes
  • Working alongside colleagues and other professionals to promote well-being, positive risk-taking, and community presence

Requirements:
