About the Role

We are seeking a highly skilled and compassionate Community Nurse to join our team at Lancashire & South Cumbria NHS Foundation Trust. As a key member of our Community Mental Health Team, you will work closely with care home staff, OTs, nurses, doctors, and the wider MDT to provide person-centered, evidence-based care to support the needs of service users in care home settings.

Main Responsibilities
  • Provide comprehensive assessments and care plans to support the complex mental health needs of service users.
  • Work collaboratively with care home staff to maintain positive health and well-being, utilizing occupation, formulation, observation, and education to promote dementia-friendly environments.
  • Undertake physical health assessments, including ECGs, bloods, and physical health observations, for shared care medication monitoring.
Requirements
  • First-level registered nurse (RNMH, RMN, or OT) with evidence of post-registration training in clinical/psychological interventions or willingness to undertake training.
  • Proven track record of clinical experience relevant to the post, including experience of working with people with dementia and mental health conditions.
  • Awareness and understanding of professional accountability, NMC or HCPC standards, and other local and national initiatives.
