About the Role

Lancashire & South Cumbria NHS Foundation Trust is seeking a skilled and compassionate Band 5 Community Registered General Nurse to join their team. As a member of the Care Home Liaison Service, you will provide person-centered, evidence-based care to support the needs of service users within care home settings.

Key Responsibilities
  • Work within the wider Community Mental Health Team to provide comprehensive assessments and care plans for individuals with complex mental health needs.
  • Collaborate with care home staff, OTs, nurses, doctors, and the wider MDT to maintain positive health and well-being.
  • Undertake physical health assessments, including ECGs, bloods, and physical health observations for shared care medication monitoring.
  • Focus on dementia-friendly environments, ensuring people have meaningful activity and support.
Requirements
  • First Level Registered Nurse RNMH, RMN, or OT qualification.
  • Evidence of post-registration training in clinical/psychological interventions/skills relevant to the post.
  • Proven track record of clinical experience relevant to the post.
