About the Role

University Hospitals of Morecambe Bay NHS Trust is seeking a skilled and dedicated Registered Nurse to join our Rapid Response Team in the community. As a key member of our team, you will play a vital role in delivering high-quality patient care and supporting the delivery of our urgent community response standard.

Key Responsibilities

  • Support the delivery of the urgent community response standard, providing a 2-hour crisis response to patients who are acutely unwell within our local community.
  • Work collaboratively with our multi-disciplinary team to develop and deliver robust programmes of care for patients and carers.
  • Undertake holistic assessments in complex and challenging situations to ensure the best possible outcomes for our patients.
  • Use your excellent communication and interpersonal skills to work effectively with patients, carers, and other healthcare professionals.

Requirements

  • Band 5 Registered Nurse adult branch
  • Post-registration education and experience
  • Scope of professional practice
  • Clinical supervision
  • Reflective practice
  • Experience in primary health care team
  • Computer literacy
