Community Mental Health Nurse

We are seeking a skilled Community Mental Health Nurse to join our team at Lancashire & South Cumbria NHS Foundation Trust. As a key member of our Care Home Liaison Service, you will provide person-centered care to individuals with complex mental health needs, working closely with care home staff, OTs, nurses, and doctors to maintain positive health and well-being.
Key Responsibilities- Provide comprehensive assessments and care plans to support the needs of service users in care home settings.
- Work collaboratively with the multidisciplinary team to maintain positive health and well-being, utilizing occupation, formulation, observation, and education to provide evidence-based care.
- Undertake physical health assessments, including ECGs, bloods, and physical health observations, for shared care medication monitoring.
- First-level registered nurse (RNMH, RMN, or OT) with evidence of post-registration training in clinical/psychological interventions.
- Proven track record of clinical experience relevant to the post.
- Awareness and understanding of professional accountability, NMC or HCPC standards, and other local and national initiatives.
