About the Role

We are seeking a skilled and compassionate Community Mental Health Practitioner to join our team at Lancashire & South Cumbria NHS Foundation Trust. As a key member of our multidisciplinary team, you will work closely with patients, families, and carers to provide high-quality care and support.

Key Responsibilities
  • Assess and manage patients with complex mental health needs, developing and implementing individualized care plans.
  • Work collaboratively with other healthcare professionals to provide comprehensive care and support.
  • Conduct risk assessments and develop strategies to mitigate risks associated with patient care.
  • Provide education and support to patients, families, and carers to promote recovery and well-being.
  • Participate in multidisciplinary team meetings and contribute to the development of care plans.
Requirements
  • Registered Mental Health Nurse with ongoing Professional Registration.
  • Evidence of commitment to continuing professional development.
  • Understanding of health and safety policies and procedures.
  • Knowledge and practice of Mental Health Act 1983.
