About the Role

This is an exciting opportunity to join our team as a Community Mental Health Practitioner, working in a busy service that provides a range of services including secondary mental health care. As a key member of our multi-disciplinary team, you will be responsible for managing a caseload of service users with complex needs, developing care plans, and delivering evidence-based interventions and treatments.

You will work closely with our clinical lead and the community mental health team to ensure that patients receive the best possible care. This role involves managing unplanned patient work, responding to escalation of risk/crisis situations, and stepping up care to a CMHT Band 6 Practitioner as needed.

Person Specification
  • Registered Mental Health Nurse with ongoing Professional Registration.
  • Evidence of commitment to continuing professional development.

  • Able to demonstrate and understand the concept of community care and the need to develop good networks.
  • Ability to maintain own performance and participation in group skills.
