About the Role

This is an exciting opportunity to join our team as a Community Mental Health Practitioner, working with adults with severe and enduring mental health needs. As a key member of our multi-disciplinary team, you will be responsible for delivering high-quality care and support to our service users.

Key Responsibilities
  • Undertake assessments and develop care plans for service users with complex needs
  • Deliver evidence-based interventions and treatments, including physical health monitoring
  • Manage a caseload of service users, working under the supervision of the clinical lead
  • Develop and maintain good networks and relationships with other healthcare professionals
Requirements
  • Registered Mental Health Nurse with ongoing Professional Registration
  • Evidence of commitment to continuing professional development
  • Good interpersonal and communication skills
  • Ability to work effectively in a multi-professional context
