Community Health Nurse

University Hospitals of Morecambe Bay NHS Trust is seeking a highly motivated and experienced Community Staff Nurse to join our team. As a Community Staff Nurse, you will be responsible for providing high-quality care to patients with a wide range of medical conditions in the community setting.
Main DutiesThe successful candidate will be required to undertake complex nursing assessments, develop and deliver programs of care for patients and carers, and work collaboratively with other professionals, voluntary sector, and informal carers to promote excellent care to patients in their own homes and community setting.
You will be joining a friendly and approachable team who are patient-focused and committed to staff development. As part of our team, you will have the opportunity to develop your skills and knowledge to the highest standard.
