The service supports people to settle into their homes safely, with a focus on older adults.

As a Post-Discharge Care Coordinator, you will be responsible for managing the delivery of this service, ensuring that all aspects of patient care are met.

You will work closely with our Community Team to provide comprehensive support to residents post-discharge.
