About the Role

We are seeking an ambitious and dedicated Support Worker to join our Higher Needs Floating Support Service. As a key member of our team, you will work with individuals who require support to maintain their accommodation and achieve positive outcomes.

Key Responsibilities

  • To engage and support clients with complex needs, including those experiencing homelessness, mental health issues, and substance misuse.
  • To develop and implement effective support plans that cater to each client's unique needs and goals.
  • To work collaboratively with local authorities, healthcare providers, and other stakeholders to ensure seamless service delivery.
  • To promote independent living skills and empower clients to take control of their lives.
  • To identify and mitigate risks associated with client support, ensuring a safe working environment.

Requirements

  • Proven experience in supporting individuals with complex needs, preferably in a floating support or outreach setting.
  • Excellent communication and interpersonal skills, with the ability to build rapport with clients from diverse backgrounds.
  • Ability to work independently and as part of a team, prioritizing tasks and managing competing demands.
  • Knowledge of relevant legislation, policies, and procedures governing social care services.
  • Strong problem-solving skills, with the ability to think creatively and adapt to changing circumstances.

What We Offer
